What exercise, and how much, does your dog get daily? (I understand. We all get busy!)
List in order the top 3 things your dog loves (ex: food, chasing bunnies, tennis balls)
What problems are you needing to have addressed in your dog?*
When your dog is having these issues, can you describe (1) what happens immediately before, and (2) what you do in response? (No worries about your answer — it just helps me to know the context, and what has been tried so far)*
This may be tricky — just do your best: what does your dog look like when they are exhibiting the behavior that's bothering you? Can you describe ears, tail, weight shift, barking, retreating/pushing forward — whatever you notice. There is no right answer!*
Has your dog ever bitten a person? If so, tell me about the circumstances. And tell me about the bite — how deep? multiple bites or one?*
Please tell me also if the dog has bitten another dog? Circumstances? How bad was the wound — just a scraping of skin, a single bite, a deep bite(s)?*
You know your dog better than anyone. How would you describe his/her character? Ex: confident/fearful, goofy/serious, anxious/easy-going, high energy/mellow, social butterfly/introvert, etc. (The dog may show different characteristics in different environments)
Is there anything else you would like me to know about your pup?

Liability Waiver*
Client agrees that he/she, his/her heirs, assignees and legal representatives will not make claim against, sue, or attach the property of Denver Dream Dog or Its Agents for injury or damage done to or Denver Dream Dog or Its Agents. Client forever releases Denver Dream Dog and Its Agents from any and all liability and demands which Client, his/her heirs, assignees and legal representatives may have or may hereafter have for injury or damage to or by dog(s), and assume all risks thereof.
- I agree to not make claim against, sue, or attach the property of Denver Dream Dog or Its Agents
Legal Waiver*
Client and Denver Dream Dog hereby agree to mediate and/or arbitrate any misunderstanding that may arise pursuant to the terms contained herein. In any action or proceeding arising out of this agreement, the prevailing party shall be entitled to reimbursement of costs and legal fees. This agreement for Boarding Services supersedes all other agreements, written or oral, previously made between Client and Denver Dream Dog.
- I agree to peacefully work with Denver Dream Dog if any problems arise
Photo and Video*
Client agrees that any pictures, audio or video taken by Denver Dream Dog or Its Agents of any private training session, group class or any other activity sponsored by Denver Dream Dog is the property of Denver Dream Dog and can be used for marketing, promotion and training purposes by Denver Dream Dog. Further, Denver Dream Dog will retain exclusive and perpetual worldwide rights to such content.
